Tribute
Tribute is a plan offered by SelectCare of Oklahoma to people with Medicare currently living in or transitioning into a custodial care facility. It offers all the benefits and services of Medicare including Medicare Part D
prescription drug coverage as well as Medicaid for those eligible.
But we also offer something Medicare and Medicaid alone do not. When you or a loved one is enrolled in
Tribute you will be assigned a nurse mid-level practitioner to help coordinate all of your or a loved one’s health
care needs.
And we offer all of this at no extra cost.

Additional Coordinated Care Services
As part of its operational design, SelectCare brings the added service of a mid-level practitioner (nurse
practitioner or physician assistant) to each of our members. The role of this mid-level practitioner is three-fold:
Assessment
- Conduct comprehensive assessments to determine member’s health status
- Visit each member routinely to ensure health status is maintained
Coordination
- Support on-going communication between long term facilities, physicians and other network providers
- Expedite authorization of services that impact member’s medical care
Prevention
- Identify potential risks to members and initiate early intervention
- Ensure timely and appropriate use of medical services
Through these activities, the mid-level practitioner serves as an extension of the primary care physician to
help maintain the health and well being of our members. Therefore, SelectCare can ensure that each member
receives the attention they need, the care they require and the comfort they deserve…all at no additional cost.
