Am I Eligible?
How to know if you qualify for membership in SelectCare of Oklahoma Tribute plans?
You’re eligible if you:
- Live in a service area served by SelectCare of Oklahoma Tribute
- Oklahoma of SelectCare Service Areas: counties: Carter,
Cleveland,
Garvin,
Jefferson,
Johnston,
Love,
McClain,
Murray,
Oklahoma, and
Stephens
- Are eligible for Medicare Part A by age or disability, and enrolled in Part B
- Continue paying your Medicare Part B. Premium.
You may not enroll in SelectCare of Oklahoma Tribute plans (in accordance with federal regulations) if you have end-stage renal disease or have had a kidney transplant within the past 36 months. You will not be disqualified for any other medical condition and will not be canceled for any medical condition you may develop once enrolled. |
